AESOP (Activities, Events, Services and Outcomes Planner) is an Electronic Health Record (EHR) developed by Help4U. With it you can capture and report information on healthcare service delivery in a click or a touch. AESOP is a single client record. It is architected to focus upon meaningful communication, incorporating the best of clinical and consumer-led practice across all aspects of the healthcare journey.
It is this attention to detail from receipt of referral, through assessment, selection of clinical pathways, care planning, visit scheduling, vitals and notes charting to completion of activity and billing that makes AESOP so attractive to healthcare organisations and their customers – their patients, staff and funders.
AESOP has been developed on the Microsoft Dynamics platform, enabling Help4U to cost-effectively customise modules suited to all types of healthcare organisations. As its namesake suggests, AESOP’s goal is to create short, succinct [patient] stories with a message. AESOP’s features are designed to keep documentation to a meaningful minimum, leaving healthcare professionals doing the job they love; patient-focussed care.

AESOP links all health professionals involved with patient care, from doctors, allied health, nurses, and support workers. This enables a clearer view of the kaleidoscope of services a patient receives, giving health professionals the peace of mind that a patient has the support needed.

Patient Stories
AESOP uses all the data collected from a patient’s support and care over time, to create a comprehensively integrated patient story. This allows health professionals to see what care was needed at what time, allowing them to know more about a patient’s health needs.

Bill of Materials
AESOP utilises a Bill of Materials, to show exactly what care input was needed, to create what outcomes. This gives health professionals the level of clarity they need, to see where care could be improved, or to create a dedicated pathway for certain conditions, based on analysis of similar cases.

Care Pathways
AESOP utilises Care Pathways from templated Bill of Materials, for specific care solutions. This enables health organisations to forecast likely costs of the care, as well as future care results. These are able to be compared to other Care Pathways, and leading research, and be changed based on changes in best practice.

Omaha System
AESOP is based on the Omaha System of care. As an internationally recognised community care model, the Omaha System utilises language similar to all organisations, allowing for greater clarity with communication between providers. Care results can also be benchmarked at a later stage.

CADI structured Documentation
Utilising look ups, and short form data entry, eliminates the need to create large patient notes via free text. This allows for the ability to analyse comprehensive big data, giving AESOP the reporting ability health professionals and organisations want.
